What Happens After You Reach Out

  1. Profile Creation – We gather essential details about you or your loved one, including name, address, date of birth, telephone number, and emergency contacts to create a personalized profile that goes into AxisCare, our customer relations management (CRM) software.
  2. Service Agreement Signature – With your approval, we will email the service agreement for electronic signature. Whether it’s you, a power of attorney (POA), or a responsible family member, this email originates from AxisCare.com and allows you to review and electronically sign the document.
  3. Initial Care Consultation – We will schedule a visit with a nursing team member to perform the initial care consultation. This comprehensive, hour-long session is crucial for gathering health history, medication details, caregiver expectations, and understanding the desired level of care. The nurse will then create a personalized care plan, ensuring that every aspect of care is considered.
  4. Desired Caregiver Schedule – We seek input from you about your preferred caregiver work schedule. Our minimum shift is 4 hours, and we are flexible about the other schedule aspects, such as shift times and days. We are happy to accommodate most scheduling needs, but we highly recommend having a set regular schedule whenever possible to ensure continuity of care. Consistent caregivers built strong relationships with clients, enhancing the overall experience.
  5. Caregiver Matching – Based on the care plan, and utilizing a special AI algorithm, caregivers who are scheduled who match your specific requirements. Our care coordination team considers factors like skills, availability, and other preferences (e.g., need for a driver, pets at home, total care, companion care, and much more.)
  6. Care Plan Communication – Once a caregiver is assigned to your case, they are given a personalized care plan created by the nurse assessor. The nurse also discusses the care plan with the caregiver to ensure preparedness and compliance. Sometimes “meet and greets” or on-site orientation for caregivers are offered to clients to familiarize them with specific tasks and to get to know the client before starting.
  7. Client Portal Access – Details about the AxisCare Client Portal will be provided, where you can access daily caregiver notes, view caregiver schedules, and review billing information.
